Human milk provides multiple biological components for metabolic and immunological programming of the infant. Milk exosomes (MEX) and their microRNA promote intestinal maturation, improve barrier function and suppress inflammation and are thus of critical importance for the prevention and treatment of necrotizing enterocolitis. Milk´s epigenetic program is only transmitted by breastfeeding or by human donor milk but is absent in infant formula, an insufficient substitute for breastfeeding.
